How the Process Works

Navigating the Self-Determination Program can feel overwhelming at first—but you don’t have to do it alone.

With experience supporting individuals and families through the SDP journey, I help bring clarity to each step of the process. Together, we build meaningful, person-centered plans, identify the right support team, and coordinate services that truly reflect each participant’s goals, strengths, and vision for their life.

A Person-Centered Plan (PCP) is the foundation of the Self-Determination Program. This plan captures what is important to the individual, including their strengths, goals, interests, and preferred lifestyle.

We work with participants and their families to develop thoughtful plans that clearly describe the supports needed to help the individual pursue meaningful opportunities and build the life they want.

For individuals interested in entering the Self-Determination Program, I provide guidance through the transition process. This includes helping families understand the steps involved, preparing for planning meetings, and coordinating communication with the Regional Center as participants move toward enrollment.

Once a participant enters the Self-Determination Program, I help organize and maintain the participant’s program and spending plan.

This includes identifying appropriate services and providers, aligning supports with the person-centered plan, and helping ensure the participant’s program remains organized and effective over time.
